Special Needs Dentistry: to treat or not to treat

There is a geographically and technically skewed workforce of specialists in Special Needs Dentistry (SND) in Australia with a high concentration of them centred in major cities. We need general dental professionals willing to manage the oral health of patients with special needs, use every opportunity to regularly assess their caries and periodontal risk, and provide adequate preventive measures and dental treatment.
General dental practitioners should be aware of the increasing complexities that patients with special needs will incur with time. Given the anticipated downward trajectory of these patients, oral health interventions need to be appropriate to the stage of their disease. Good communication is also fundamental to good clinical practice. Therefore, dentists need to have knowledge of the available strategies to communicate effectively with these patients. It is therefore important that patients that can be managed by general dentists continue to receive dental care in the general dental practice. However, to ensure continuity of care, it is important that dentists understand where and how to refer patients to specialists in SND, and plan ongoing dental care after referred services.

Laser Radiation Safety and Compliance to State Regulations

Meeting the legislative requirements (licensing, procedures, and compliance) for a laser radiation practice can be daunting, not to mention a frustrating process. There aren't clear ‘road signs’ or directions to help guide what to do. Indeed, one can waste substantial time understanding these legal requirements, let alone applying them to practice. Of course, you still require plenty of time and energy to run your business/practice.

All practices with a laser machine must comply with the relevant local radiation health regulatory requirements for radiation safety. The core purpose is to ensure the safety of everyone, not only for the people operating the laser machine but also for people within proximity of the laser machine/exposure. As you know, laser radiation energy can be invisible with significant impact on health and wellbeing if not used correctly.

Each state and territory of Australia have their radiation safety guidelines and requirements. They differ both in applications and implications.

• It is crucial to follow all the correct procedures and processes for your radiation practice before operating laser machines. If you operate laser practices across Australia, be mindful of the different sets of legal requirements and compliance processes that you must adhere too.

• Ensure that you have all the necessary licences, compliance certification, ongoing radiation safety and protection procedures for your laser practice. Getting it right from the onset can ensure that you are confident come time for an audit and inspection visit from the radiation health regulator

• Make sure your laser practice maintains ongoing radiation compliance – Any practice/business owners do not wish to have their laser practice/operation interrupted if they are issued with an improvement or action notice by the radiation health regulator

• Engage an experienced radiation consultant from the onset for your laser practice. The radiation consultant can also assist with ongoing laser radiation compliance for your laser practice to avoid significant penalties by the radiation health regulator for non-compliance and address any radiation-related matters (including relocation, sale, purchase, disposal,decommission, radiation incidents etc.)

How Do We Ever Know the Side Effects of Drugs?

All drugs have the potential for side effects. Some drugs cause many side effects and others hardly any. Most of us assume that drugs have been subjected some sort of testing before they are marketed, but this does not always happen.

Clinical trials can elucidate common and predictable adverse effects, but inherent weaknesses in clinical trial design limit what they can tell us. Such limitations include a small number of trial participants, that trial subjects are often different from the drug’s target users and that groups such as children, the elderly, women of childbearing age, are excluded from clinical trials.

Old drugs in use today, such as sedating antihistamines, may have never been subjected to clinical trials. Over-the-counter products and herbal remedies are not required to undergo clinical trials at all before marketing. So how do we ever know the side effects of drugs? And how do we know these drugs are safe? The answer is Pharmacovigilance, which will be explained in this presentation, as well as the role dentists play in this important endeavour.

Cracked teeth are unfortunately a common concern in dental practice, and management of the various types of cracks can present significant challenges for the clinician. The use of complex indirect restorations can provide a viable solution in some cases, but the status and strength of the underlying tooth structure are often questionable. Long-term outcomes are often unpredictable and ultimately catastrophic failure may occur with subsequent loss of the tooth. Alternative options for restorative management are available using newer formulations of short fibre reinforced composite (SFRC) materials, which have been shown to be very successful in strengthening tooth structure, reducing crack propagation, and increasing the overall fracture toughness of teeth. Restorative techniques can be utilised to strengthen and restore both vital and endodontically treated teeth, and these can be used alone or in combination with an overlying direct or indirect restoration. This presentation will look at the various presenting situations involving cracked and compromised teeth and discuss strategies for managing and improving the successful outcome of restorative treatments.
